Affiliate Instructions


New Affiliate Account

Step 1

  • Go toÌýMyUTÌýportal and log in.
  • Select the link underÌýTechnology Services and Software Information.
  • Click on link titledÌýAffiliate Request/Update of UTAD & Rocket #.

Step 2

  • Log in to theÌýAffiliate Demographic Entry SystemÌýwith your UTAD username and password.

Step 3

  • SelectÌýAdd New Account.
  • Add New = the individualÌýdoes not have a current Rocket NumberÌýand has never been associated with the University in the past.Ìý Is not a former student, recruited student, employee, or affiliate.

Step 4

  • Enter the persons first and last name and maiden name (if applicable)
  • Enter the date of birth and the social security number.
  • Enter the last date the affiliate will be on campus or will need access.Ìý Affiliate codes are valid forÌýone year.
  • Enter affiliate company name which could be business, university, high school, etc. of the affiliate coming to campus.Ìý (this must be completed, even if the person is self-employed.)
  • Enter the affiliate code from the drop down list and choose the most appropriate code.Ìý If you do not see a code that applies to you, please contactÌýaffiliaterequests@utoledo.edu.ÌýMake sure you are submitting all theÌýcorrectÌýinformation.Ìý If any information is not correct, you will be asked to re-submit your request.
  • Enter the person'sÌýhomeÌýaddress.
  • Comments:Ìý You can add theÌýperson's titleÌýhere.
  • Click submit when finished.

Two email confirmations

  • Once the account has beenÌýupdated, the requestor will receive an automated email with the subject: Request for Affiliate Update Received.
  • Once the account has beenÌýcreated, the requestor will receive an email with the affiliate's Rocket Number and instructions that will need to be communicated to the affiliate.

UpdatingÌýan Existing Affiliate Account

Step 1

  • Go toÌýMyUTÌýportal and log in.
  • Select the link underÌýTechnology Services and Software Information.
  • Click on link titledÌýAffiliate Request/Update of UTAD & Rocket #.

Step 2

  • Log in to theÌýAffiliate Demographic Entry SystemÌýwith your UTAD username and password.

Step 3

  • SelectÌýUpdate Account Existing Account.
  • Update Existing Account = the individual has a current Rocket Number.
  • Enter Rocket Number.Ìý This will return the affiliates demographic data.Ìý (automatically populated is the rocket number, first name, last name, and date of birth).

Step 4

  • Enter affiliate company name which could be business, university, high school, etc. of the affiliate coming to campus.Ìý (this must be completed, even if the person is self-employed.)
  • Enter the affiliate code from the drop down list and choose the most appropriate code.Ìý If you do not see a code that applies to you, please contactÌýaffiliaterequests@utoledo.edu.Ìý Make sure you are submitting all theÌýcorrectÌýinformation.Ìý If any information is not correct, you will be asked to re-submit your request.
